The question this issue poses
A cardiac-implant maker whose profit went from a ₹7.35 Cr loss in FY24 to ₹69.08 Cr in FY26 is asking 51.57× those FY26 earnings — in a pure offer for sale that hands the company nothing, so the entire question is whether FY26 is the new base or the peak.
IPOVista score
Every pillar opens to show the raw metric, the curve it was mapped through, the sub-score and its weight. Disagree with the weighting if you like — the arithmetic is all here.
IPOVista score
A trade-off in both directions
83% of the scoring model was backed by disclosed data for SMT, giving medium confidence. Weights for anything missing were redistributed rather than scored as zero.
Below the line: market sentiment. Capped at 8 of 100 points so a hot grey market can tilt a score but never carry one.

Entirely an offer for sale
None of the money raised reaches the company. Existing shareholders are selling down; the balance sheet is unchanged by the listing.
Grey market data is unofficial
GMP is an unregulated, unverifiable over-the-counter quote. It is shown as a sentiment indicator and carries no settlement guarantee.

The company
Founded 2001 · Surat, Gujarat · 1,605 employees
SMT makes the small implantable devices that cardiologists thread through a patient's arteries instead of opening the chest. It was founded in Surat in 2001 by Dhirajlal Kotadia and sells two families of Class III (EU) / Class C-D (India) devices. Vascular Intervention covers drug-eluting coronary stents, PTCA balloons and drug-coated balloons — its flagship is Supraflex Cruz, a biodegradable-polymer sirolimus stent. Structural Heart covers Hydra TAVI replacement aortic valves and Cocoon occluders, the plugs used to close holes in the heart. A third bucket carries renal stents, peripheral drug-coated balloons and traded goods. The company says it was the first in the world to win a CE certificate for a biodegradable-polymer paclitaxel-eluting stent (Infinnium, December 2005). It manufactures at three plants — Surat SEZ in Gujarat, Hyderabad in Telangana, and Nonthaburi in Thailand — and runs two R&D centres with 156 professionals, holding 119 granted patents and 78 pending applications. The Frost & Sullivan report commissioned for the offer puts its share of India's drug-eluting-stent volume at roughly 25% in FY26, with top-five volume positions in DES in Germany, Spain, Poland and Brazil and in occluders in Thailand, South Korea and India.
SMT sells devices, and the mix is shifting toward the higher-value end. Vascular Intervention fell from 68.57% of revenue in FY24 to 61.14% in FY26, while Structural Heart — TAVI and occluders — rose from 12.26% to 16.77% and Others from 19.17% to 22.09%. Geography matters more than product here: India was only 26.13% of FY26 revenue against Europe at 38.66% and rest-of-world at 35.21%, so roughly three-quarters of sales sit outside India's NPPA price-capped market. Routes to market vary by country — direct operations in Germany, Spain, France and the UK, a hybrid model in India selling to large hospital groups, and distributors elsewhere. Growth has come partly from buying the distribution and the technology: Imex in Spain and Zarek in Brazil converted distributor relationships into direct and hybrid models, while Vascular Concepts and Vascular Innovations bought the company into occluders and TAVI. The cost base is import-heavy — 77.18% of FY26 purchases were imported, mainly from Germany, China, the Netherlands and Ireland — and R&D runs at a consistent 8-9% of revenue, ₹104.16 Cr in FY26.
What makes it hard to copy
SMT holds about 25% of India's drug-eluting-stent sales volume (FY26, Frost & Sullivan) and is a top-five DES player by volume in Germany, Spain, Poland and Brazil, backed by 119 granted patents and its own CE-certified plants — a regulated-Western-market position almost no other Indian device maker has built.

The numbers
Revenue has compounded at 15.9% a year across the 3 reported financial years.
Figures in ₹ crore, as reported in the prospectus. Interim quarters are not annualised.
Profit and operating margin as a percentage of revenue
At each period end, in ₹ crore. Gearing below each period is borrowings over net worth, from these same figures.
Profit after tax and net cash from operating activities, in ₹ crore. The figure under each period is cash generated per ₹100 of profit.
Above 100% the business collects more cash than it books as profit. Persistently below it, growth is being funded out of working capital.
The price
At the upper band this issue asks a 15.4% premium to the median P/E of the listed peers its own prospectus names.

Demand
Subscription is hard exchange data. Grey-market premium is not — it is an unofficial quote, shown because readers want it and labelled because it deserves the label.
Cumulative bid multiples
Bidding has not opened yet, so there is no subscription data to show.
Premium quoted over the ₹344 cut-off price. Unofficial and unregulated — sentiment, not a forecast.
The grey market is an informal, unregulated market with no clearing house and no obligation to settle. A premium here does not guarantee a listing gain, and quotes can move sharply — this one ranged ₹40 to ₹40, a spread of ₹0, across the period shown.
Work it out
Three calculations, each stating the assumption it rests on.
Shares are applied for in whole lots at the cut-off price. The full amount is blocked in your bank account under ASBA, not debited, until allotment.
One lot is 43 shares at ₹14,792. Retail applications are capped at ₹2,00,000, which allows up to 13 lots here — apply for more and you are bidding as a non-institutional investor instead.
Applying needs a demat account with a linked UPI ID, and the ₹14,792 is blocked rather than debited.

When the retail category is oversubscribed, SEBI requires a lottery for the minimum lot. Every application has the same chance, whatever its size.
Retail subscription figures are not available yet, so there is nothing to estimate from.

Grey-market premiums have historically moved sharply in the days before listing and have been wrong in both directions. Treat this figure as a measure of current enthusiasm, nothing more.
